All families are required to submit an official copy of their child’s current immunization record to MSD. Immunization records for new students must be on file in the office prior to the child's start at MSD. Students must have complete immunization records, based on their age. MSD does not accept medical or religious exemptions. Families are required to submit updated immunization records as their child ages while enrolled at MSD. Records must be in the North Carolina-approved format, which includes the NC state seal faintly visible in the background. If your child has already received the required school vaccinations, there is no need to schedule a new appointment. Your healthcare provider can generate the official record based on your child’s file. Other health information is captured in the forms families complete in late spring/summer. Families complete medication administration permission forms when dropping the medication at the front office. It is helpful when families submit the completed FARE allergy form along with any emergency medication.
